To set an appointment, please call the patient service line at 574-264-4952 or toll free at 888-239-9677, Monday through Friday from 8:30 a.m. to 5:30 p.m.

Please be prepared for your appointment

If it’s your first appointment at OSMC, please arrive 15-20 minutes early to complete the registration process. If you’re filing insurance through Workers’ Compensation, we will require proper authorization from your employer. If any medical reports exist that pertain to your condition, please have them faxed or mailed in advance. Also, please have any relevant X-rays mailed in advance to OSMC.
